BASIC FUNCTION:
Plans, organizes, and coordinates programs of activities in and outside the care center which will provide entertainment, inter-communications, exercise, relaxation, opportunity to express creative talent and fulfill basic psychological, social and spiritual needs to all residents. This position is classified as Hourly, Non-Exempt.
QUALIFICATIONS:
- Must have a high school diploma or GED.
- Prefer to be certified and licensed by the State of Colorado or a graduate of a CNA certification class, and become certified within 120 days of hire.
- Experience in activity programming in a long term care setting is also preferred.
PHYSICAL DEMANDS
- Must be able to crawl, kneel, squat, bend, lift 40-60 pounds, and walk up and down halls all during the eight hour shift to give care to residents.
- Must have adequate hand grip and flexible arm and shoulder joints for lifting and turning residents, as well as lifting them into a chair, to ensure safety of the resident in the job performance.
- Must be able to escort residents in wheelchairs to and from activities and meals, as well as rides outside.
- Must be able to stand for 10-20 minutes while bathing residents.
- Must be able to bend for 5-10 minutes for bed making and dressing residents.
- Must be able to safely handle hazardous chemicals.
- Hearing must be adequate to be able to hear residents’ complaints, needs, desires, call lights, alarms, calls for help and to answer telephone.
- Vision must be adequate to observe residents and their condition, observe call lights, complete documentation on paper and computer, dial telephone, push wheelchairs, linen carts and ambulate from room to room.
- This position is classified in Category 1 of infection control.
- This position could require the performance of any duty that involves the exposure and/or potential exposure to blood, body fluids or tissue.
REPORTING RELATIONSHIP: Reports to the Activity Director and the Administrator.
AUTHORITY:
- Has the authority to make decisions concerning the residents care plans, budget, and activity calendar when the Activity Director is unavailable.
- Has the authority to give direct resident care if employee is a Certified Nursing Assistant, as outlined in the Nurse Aides Practice Act.
- Has the authority to access the electronic health record according to the HIPAA Privacy and Security policy as specified under clinical authorization.
RESPONSIBILITIES:
- Responsible for meeting the social, psychological and recreational needs of the residents.
- Responsible for planning and organizing activities geared to the residents’ preferences and needs.
- Responsible for evaluating residents and setting realistic goals according to their level of functioning and personal preferences.
- Responsible for having current driver’s license and maintaining a good driving record.
- Responsible for covering daytime, evenings, weekends and holidays as requested.
- Responsible for 1-1 programming visits.
- Responsible for safely operating and maintaining activity equipment i.e. ceramic kiln, popcorn machine, facility vehicles and lifts and etc.
- Responsible to keep the Certified Nurse Assistant License current if applicable.
- Responsible to use universal precautions and necessary personal protective equipment (i.e. goggles, gloves, mask) to protect residents, yourself and others from exposure to blood, body fluids and tissue; wash hands after each resident contact and remove and dispose of gloves after each resident contact.
- Use hand sanitizer when hand washing isn’t available.
- Responsible to communicate with the resident while assisting or doing care for the resident.
- Responsible to report any resident abuse to the authorities and the Director of Nursing as mandated under Senate Bill 13-11 and per the facility’s resident abuse policy.
- Responsible to report incidents or exposure to the Activity Director or supervisor on duty.
- Responsible to follow all rules and regulations of the facility.
- Responsible to maintain a neat and clean appearance.
- Responsible to attend in-service, professional workshops and staff meetings at the request of the Activity Director.
- Responsible to treat residents and staff with kindness and consideration, and to promote an atmosphere of cooperation and teamwork with the supervisors and employees of all other departments.
- Responsible for the care and stewardship of all company assets.
- Responsible for the care and stewardship of all residents’ personal belongings.
- Responsible for protecting the confidentiality of resident information and reporting all suspected violations of the (HIPAA) Privacy or Security Policy to the Privacy Officer or Security Officer.
- Responsible for promoting excellence in person centered care by supporting the residents’ individuality through personal choices of daily life.
- Responsible to help residents preserve as much independence as possible and to maintain their highest possible level of functioning.
- Responsible to comply with attendance and timeliness standards.
- Responsible to maintain a healthy, clean and safe environment, free from fall hazards, for the residents and staff.
- Responsible to participate in emergency operations as need arises and as instructed by the Incident Commander.
ESSENTIAL JOB FUNCTIONS:
- Plan, organize, and coordinate a program of activities for residents needing 1 to 1 supportive and maintenance activities.
- Encourage participation in 1 to 1 activities, and provide materials and supplies in keeping with the residents’ financial status and departmental budget.
- Assist residents with obtaining personal needs items by assisting them with shopping.
- Assist residents with obtaining their PN money from the office and/or signatures on the PN book after purchases are made.
- Make resident shopping list a week in advance, and obtain approval from the Activity Director.
- Make purchases for the activity department with prior approval from the Activity Director; present receipts to the Director for signature.
- Assist nursing services with residents’ appointments if needed.
- Conduct and/or assist with group activities.
- Provide reading materials, in conjunction with library services, for all residents, including Braille books and/or tapes for the visually impaired.
- Contact relatives and act as a liaison between residents and relatives, at residents’ request.
- Document daily participation in activities and 1 to 1 visits.
- Prepare and submit reports as directed by the Activity Director or the Administrator.
- Supervise and orient new employees and volunteers in the absence of the Activity Director.
- Attend funerals at the request of the Activity director or the Administrator.
- Pay for own meal at outings and submit receipt for reimbursement.
- Participate in Resident Council meetings at the request of the residents, and keep minutes for meeting as the residents’ request.
- Provide support for connection and meaning in the life of the residents through companionship, spontaneity, variety and opportunities to give and receive care for each other.
- Encourage resident participation in activities of residents’ personal preferences as outlined in the residents’ plan of care.
- Manage resident interactions during activities and intervene if necessary.
- Report and/or record any changes observed in condition or behavior of the resident; report all unusual conditions, reactions or complaints of residents to nurse using the “Stop and Watch” tool unless it’s an emergency situation, then notify nurse immediately.
- Provide resident privacy by knocking on doors or ringing doorbells and greeting residents prior to entering room and pulling curtain or closing door when rendering personal care.
- Promptly answer call lights.
- Anticipate residents’ needs, check on residents regularly and demonstrate a cooperative attitude when assisting residents and when communicating with family members.
- Make sure residents’ call cords are within reach at all times when leaving a room.
- Ensure that any chemicals used are properly labeled and locked up when not in use.
- Perform C.N.A. duties when requested by the Activity Director or Administrator.
