Job Description:
Under the supervision of the Co-Response Program Manager, the Case Manager supports individuals experiencing behavioral health crises through field-based case management, care coordination, and advocacy in collaboration with law enforcement and Co-Response Clinicians. The role emphasizes crisis resolution, diversion from the criminal justice system, and linkage to long-term care and community support. This position works with a diverse population including children, adults, and older adults, applying trauma-informed, culturally responsive, and client-centered practices. The position will work in Chaffee County and may need to travel. The position requires a Bachelor's degree in Human Services, Psychology, Criminal Justice, Social Work, or a related field. The position works in Chaffee County, is full time non-exempt, with an hourly wage range of $24.04 to $28.85, DOQ.
