Job Description:
The Leadville SBHC Patient Access Representative is the first point of contact between the students and their families and our School Based Health Center, with responsibilities for supporting the students and their families either in person or via the phone, offering a professional invitation to our clinic while upholding the ASQ (Access, Stewardship, and Quality) pillars. This will include scheduling, patient outreach, and ensuring care is coordinated and integrated between all three service lines (medical, dental, and behavioral health).

The work environment is in a healthcare setting, which can be fast-paced and involves working directly with patients and healthcare professionals. The environment is primarily clinical, but the individual may work in a clerical setting. Many of our patients with chronic disease have a minimal level of health literacy and a significant number of barriers to accessing health care.
